Louann Jeanne Hammond, 34, of Nashville died Tuesday December 6, 2016 in Tift Regional Medical Center.
She was born April 10, 1982 in Macon, GA to Lewis Martinez and the late Alethia Elizabeth Lloyd. She was a homemaker.
She is survived by her husband: Carl Hammond; 4 daughters: Moon Martinez, Dakota Martinez, Chayanne Hammond, and Sky Lynn Hammond; 1 son: Carl Hammond, Jr.; 4 sisters: Christina Martinez, Witcha Martinez, Alisia Martinez, and Katalina Martinez; and 4 brothers: Hosea Martinez, Ricco Martinez, Jackie Ray Strickland, and John T. Martinez.
The family will honor her request for cremation and no formal services will be held.
